Where can I send my medical history?
We will provide intake forms and assessments for you to complete prior to your initial consultation, which are usually enough for your provider to diagnose and present treatment plan options to you. Having your medical history and notes will be handy while filling out our intake forms. It will also be useful to present to your provider during your appointment. If they request for you to forward your medical history to us, we will provide further instructions on how to do so.
